Abstract

Over the past decades, public trust in medical professionals has steadily declined. This decline of trust and its replacement by ever tighter regulations is increasingly frustrating physicians. However, most discussions of trust are either abstract philosophical discussions or social science investigations not easily accessible to clinicians. The authors, one a surgeon-turned-philosopher, the other an analytical philosopher working in medical ethics, joined their expertise to write a book which straddles the gap between the practical and theoretical. Using an approach grounded in the methods of conceptual analysis found in analytical philosophy which also draws from approaches to medical diagnosis, the authors have conceived an internally coherent and comprehensive definition of trust to help elucidate the concept and explain its decline in the medical context. This book should appeal to all interested in the ongoing debate about the decline of trust - be it as medical professionals, medical ethicists, medical lawyers, or philosophers.

Abstract

This book provides an up-to-date description of cross-cultural aspects of end-of-life decision-making. The work places this discussion in the context of developments in the United States such as the emphasis on patient informed consent, “right to die” legal cases, and the federal Patient Self-Determination Act. With the globalization of health care and increased immigration from developing to developed countries, health care professionals are experiencing unique challenges in communicating with seriously ill patients and their families about treatment options as well as counselling all patients about advance medical care planning. While many Western countries emphasize individual autonomy and patient-centered decision-making, cultures with a greater collectivist orientation have, historically, often protected patients from negative health information and emphasized family-centered decision-making. In order to place these issues in context, the history of informed consent in medicine is reviewed. Additionally, cross-cultural issues in health care decision-making are analysed from the perspective of multiple philosophical theories including deontology, utilitarianism, virtues, principlism, and communitarian ethics. This book explores the growing intellectual interest in the politics of immunity. It argues that taking an ‘immunitary perspective’ is necessary if we are to better appreciate the body as a site of politics in the contemporary age. It explores the dynamic tensions between community and immunity, belonging and fragmentation, the social and the individual. Abstract

This book brings into sharp relief the extent to which the medical profession has enabled or participated in actions that are at moral crossroads. Physical and psychological abuse and violations of medical codes have already been brought to light by concerned bioethicists responding to ethical lapses of the “war on terror.” This book goes to the next level by looking at three areas that also merit our attention and call us to speak out against abuses. These are (1) dehumanization (such as forced nudity, hooding, sensory deprivation, exploitation of phobias, waterboarding, and environmental manipulation), (2) non-consensual forced-feeding, and (3) solitary confinement. Each area raises important questions for the medical profession. Author Wanda Teays calls upon doctors and nurses to reflect on the role they play in the unethical treatment of prisoners and detainees by crossing moral boundaries around each of these areas. In the process, we are reminded that bioethics is global, not local — and the concerns of the discipline encompass issues with a wider scope. .
